1. 1. High School transcript of grades. You do not need to submit high school transcripts if you have already graduated from high school and are currently attending college.
2. 2. College or technical school transcripts if currently enrolled. If you are on the semester system, please have your school send a transcript directly to the foundation following the end of the spring semester.
3. 3. Copy of financial aid letter from the school you will be attending. The scholarship committee expects all eligible applicants to apply for financial aid in addition to the Kids' Chance Scholarship. It is important to apply early. See the next page for tips.
4. 4. Letters of recommendation (optional).
5. 5. Please provide proof that parent has been found permanently and totally disabled from an on the job injury. For example, a copy of a court order or statement from the workers' compensation insurance carrier. A doctor's statement or Social Security Administration statement are not sufficient.
6. 6. Death Certificate of deceased parent (if applicable). Death must have occurred as the result of an on the job injury.
7. 7. Please attach a brief description of the accident.
8. 8. Copy of last year's tax return of parent or guardian. The first page showing adjusted gross income is sufficient.
Please list the names of all persons who assisted the applicant in preparing this document.
List one per line.
The amount of each Kids' Chance Scholarship awarded is decided by the Scholarship Com¬mittee. Scholarships have traditionally ranged from $500 to $3,000 in amount. No scholarship will exceed the annual cost of tuition and books at the most expensive in-state public univer¬sity.

The deadline for submitting completed applications and supporting documents for the upcom¬ing school year (beginning in August or September) is May 1 of each year. Applications must be received in the foundation's office by May 1, and will not be accepted after the deadline. Scholarships will be awarded in late June or early July. Please send to:
